Donation Form Explanations
Single Donation:
Please use this form when sending in any donation (collective or individual) other than the Direct Debit form (see below).
- If you are collecting money on behalf of a group make sure you indicate that on the form and include all Gift Aid forms along with the donations.
- If you are sending in money collected from a sponsored event, include this form along with your sponsorship form and all donation monies.
- If you are making a one-off donation make sure you indicate that you are donating on behalf of yourself and remember to fill out the Gift Aid declaration if you are a UK taxpayer.
Sponsorship Form:
For any sponsored events or activities use this form to collect your donation amounts and gift aid declarations. Make sure that whoever is sending in the sponsorship form and donation includes a single donation form as well.
Gift Aid forms:
If you are a UK taxpayer and if handing your donation to your treasurer you will need to fill in a Gift Aid form, however you will only need to do this with your first donation, for any further donations made your Gift Aid status will be applicable. This allows CAFOD to claim back 28% from the government which can make a significant difference to the amount of money you donate.
Direct Debit:
Please use this form if you wish to make regular donations to the Centenary Appeal by Direct Debit. By filling in this form money will be transferred from your account to CAFOD on a regular basis. You can specify on the form below on which date the money is donated and remember to Gift Aid it if you are a UK taxpayer.
The ‘Give Water, Life and Hope’ appeal runs for one year. If you would like to stop giving to CAFOD when the appeal ends, please ensure you cancel your Direct Debit on or after April 2009. Payments received after the appeal closes will go towards CAFOD’s general funds.
Please note, if you give using this method, the money cannot be attributed to your Circle.
